Why oral semaglutide has strict timing rules

Absorption is the whole problem. The tablet has to be taken on an empty stomach, first thing, with no more than a small amount of plain water, and then nothing else for a set interval afterwards.

Break that and absorption drops, which means the dose you took is not the dose you received. People who find the tablet ineffective have very often been taking it with coffee or breakfast.

How oral GLP-1 treatment works day to day

  1. Same time daily

    First thing, before anything else. Consistency matters more here than with a weekly injection because a missed window is a missed dose.

  2. Plain water only

    A small amount, nothing else. Coffee, juice and food all interfere with how much of the dose is actually absorbed.

  3. Then wait

    A set interval before eating, drinking anything else, or taking other medication. Your clinician will give you the exact figure.

  4. Titrate as usual

    Doses step up on a schedule the same way as the injectable, conditional on tolerance.

Needle aversion is the obvious one and it is a real clinical consideration rather than squeamishness. A treatment you avoid taking does not work.
