Side effects

GLP-1 side effects, and which ones actually matter

Most people on a GLP-1 get some digestive upset, most of it in the first weeks and most of it manageable. A much smaller set of symptoms are not manageable and mean stopping and getting seen. Knowing which is which before you start is the point of this section.

  • Mostly digestive, mostly early
  • Red flags listed plainly
  • Hold the dose rather than push

Common vs serious

Sorting GLP-1 side effects into two piles

The common pile is digestive: nausea, fullness, constipation, reflux, sometimes diarrhoea. It is usually worst in the days after a dose increase and eases as your body adjusts. Most of it responds to slowing the titration, changing meal size and timing, and simple measures your clinician will walk you through.

The serious pile is short and specific: severe persistent abdominal pain, repeated vomiting you cannot keep fluids down through, signs of gallbladder trouble, or symptoms suggesting pancreatitis. These are not "push through it" symptoms. They mean stop and be seen.

Separately there are interactions and effects that need watching rather than fearing: blood sugar if you take diabetes medication, mood, and the muscle you lose alongside fat if protein and activity are ignored. Common side effects, Serious side effects and Nausea and digestion go further into it.

How they are handled

What happens when a GLP-1 side effect appears

  1. You report it

Between appointments as well as at them. Side effects reported early are much easier to manage than side effects endured quietly for a month.

  2. It gets triaged

Common and expected, or a red flag. That distinction decides everything that follows and it is a clinical judgement, not a web search.

  3. The dose is held or lowered

Most digestive side effects settle if the current dose is held rather than pushed higher. Going back a step is a normal move, not a failure.

  4. Or treatment stops

Where a serious effect appears, medication is stopped and you are directed to appropriate care. That is the whole reason supervision exists.

Common questions

GLP-1 side effect questions, answered

How long they last, when to worry, and whether the dose has to change.

Digestive side effects are usually worst in the first days after starting or after a dose increase, and ease over the following one to two weeks. If they are not easing, the dose is usually the thing to adjust rather than something to endure.
Severe abdominal pain that does not pass, repeated vomiting where you cannot keep fluids down, or symptoms of gallbladder or pancreas trouble. These mean stopping and seeking care the same day rather than waiting for your next appointment.
Often, yes. Holding at the current dose or stepping back one level resolves a great deal of digestive upset. That is a standard adjustment and it is what a review schedule is for.
It can. Diabetes medication is the most important, because a GLP-1 alongside insulin or a sulfonylurea can drop blood sugar too far. Thyroid medication and anything with strict absorption timing also matter. Bring your full list.
Some lean mass loss happens with any substantial weight loss. Protein intake and resistance activity are what limit it, and they are part of the plan rather than an optional extra.

Third week was rough. They held the dose instead of pushing it up and the next fortnight was completely different.

I messaged about pain at 11pm expecting nothing until Monday. Got a call back the next morning and was told to stop and get checked, which was the right call.

The advice about meal size was more useful than anything I found online. Small and often, and it stopped being an issue.

My diabetes medication needed changing at the same time. That got picked up before I started rather than after.
